If you would like to request an appointment, please fill out our new patient form. Once we receive your form we will contact you within 5 business days.

For refill requests please call 1855-874-7763 and choose option 5. Once we receive your request please check with your pharmacy for the status of your refill in 48hs.

To cancel appointments, please use your patient portal or call 1855-874-7763 option 3.

Outside of usual business hours (9am-6pm Monday-Friday), if you are in crisis, you can utilize the ReSolve Crisis Network. They have people available for you to speak with 24 hours a day, 7 days a week. They also have Mobile Crisis Teams that can come to your home.
